From Clinic to Community: Health Care on the Front Lines is a blog managed and maintained by the University of Minnesota Department of Family Medicine and Community Health. The blog features news updates and information about family medicine and primary care at the University of Minnesota and beyond. 



University of Minnesota Family Medicine and Community Health was one of the first family medicine departments in the U.S., created in response to a legislative mandate to train primary care physicians. For more than 45 years, we have worked to transform and renew family medicine through education, research, and patient care.

Education

We have an integrated education model for family medicine, following the continuum from medical school through residency and beyond. Our resident, medical student, fellow, and continuing education activities focus on innovative didactic and practice experiences.  Learners are well prepared for future practice in family medicine and primary care.

Research

We are actively engaged in family medicine and primary care research, working to speed the uptake of innovations into primary care practice. Our research is organized into three main areas: health disparities, healthcare delivery, and population health. We are consistently ranked among the top National Institutes of Health-funded family medicine departments, with faculty ranked in the top 20 of principal investigators.

Patient Care

We have embraced the health care home model of care and modified our practices to include a team-based approach, integrated mental health and pharmacy services, and patient complexity assessment. We promote the Institute for Healthcare Improvement's Triple Aim healthcare framework by providing high quality clinical care and improving patient satisfaction in an evidence-based, cost-effective way. We also strive for a fourth aima good provider experience.
